After more than a decade of advocacy, research, and investigation, PULSE Families and Survivors for Justice has released Preventable Failures: A New Critical Incident Review of the PULSE Nightclub Shooting.
This report compiles years of public records research, documentary evidence, witness accounts, and independent analysis into a single resource for the public, journalists, policymakers, and researchers.
The report examines evidence related to building code compliance, regulatory oversight, the emergency response, government transparency, and the onePULSE Foundation, while documenting questions that the authors argue were never fully investigated. Its purpose is to preserve the historical record, promote accountability, and encourage informed public discussion based on cited evidence.
We invite you to read the report, review the evidence for yourself, and draw your own conclusions as we work with Representatives Anna V. Eskamani and Carlos G. Smith to advance legislation establishing a PULSE Truth and Accountability Commission dedicated to conducting an independent review of the events surrounding the PULSE tragedy and ensuring transparency and accountability for victims, survivors, and the broader community.
